  1. If you are ready to schedule an appointment, please complete the information below first.

  2. Next, click the “Schedule an Appointment” button to the right, and select from one of the open time slots that you want to secure. Complete the required information to save your time slot.

  3. Once an appointment has been made, confidentiality and intake forms will be emailed to you. These forms are expected to be returned before your appointment.

  4. Payment is made online before your appointment in the payment portal. There is a 48-hour cancellation policy for a full refund.

  5. If you do not see an available timeslot that works for you or have additional questions before scheduling an appointment, please only complete the information below, and you will be contacted during ministry hours.
